Registration due: April 15, 2015 - Extended to May 1st, 2015
Registration fee: $300 for faculty, $250 for fellows, $200 for students
Included in registration fee:
- Opening reception on Monday, June 8th 2015
- Refreshment breaks and lunches for June 9 – 11
- Street Car and Tram tickets
- Banquet/ social outing on the evening of Tuesday, June 9, 2015
- Program booklet
Guest ticket for Tuesday social outing will be extra – $100.
Cancellation: Cancellation is possible (less a processing fee) if done by May 25, 2015. No refunds are possible after this date.
Payment is by credit card via the secure link that is now available above (a small processing fee will be charged). Or a check made payable to "OHSU Foundation" may be mailed to:
